The Federal University of Health Sciences, Azare (FUHSA) Remedial Admission Form has been released for the 2024/2025 academic session.

This is an opportunity for candidates to prepare for admission into FUHSA and other universities.

Available Programmes at FUHSA

FUHSA offers remedial studies to prepare candidates for various health science programmes, including:

To be eligible, you must have at least three credit passes in O’ Level subjects.

These must include English Language and any two other science subjects, such as Biology, Chemistry, or Physics, in one sitting.

You must also register for the 2025 JAMB (UTME) and achieve the required score.

If you are deficient in any subjects, you must register and pass them at the credit level in SSCE or NECO.

Candidates awaiting O’ Level results can also apply.

Follow these steps to apply:

Create a profile on the FUHSA Remedial Admission Portal and fill in your bio-data.
- Use a valid email address as all communications will be sent via email.
Pay the non-refundable application fee of ₦10,000.
- Payment can be made online or by generating an RRR and paying at any bank.
- Ensure payment is made through your profile, as only payments made this way are valid.
Fill out the application form after making a successful payment.
Print out the completed application form for future reference.
